A problem statement (or statement of need) is a key component of a health promotion program plan. This assignment provides the evidence based rationale for program need and justification for program activities in the health promotion program plan and proposal. It is expected to identify a gap in the community reality and the goal for health. Problem statements use focused, clear, and credible writing to explain a specific health problem that exists among a group of people in a community, including the identification and discussion of relevant disparities.
Ideally, problem statements utilize both primary and secondarycommunity assessment data; however, in this course you will solely use secondary data. Secondary data refers to existing data collected for other purposes, that you may use to complete the community assessment within the course semester time frame. Some examples of secondary data are peer reviewed journal articles, government publications and reports (federal, state, and county), and organizational publications. Your data gathering process will follow the first three phases of the PRECEDE-PROCEED model, and the data collection guide will be your research roadmap. Your final 4-5 text page paper (not including title and reference pages) should address each of the four parts described below.
Part I: The Nature and Extent of the Need/Problem
Using data obtained from your social assessment research, describe the community.

Identify who is and who is not affected by the health problem. Is the local community experience different than other communities around the state/nation/world?

Correct use of spelling, grammar, and syntax. Students are expected to edit problem statements for proper grammar, spelling, and syntax. Errors will be noted once and all subsequent errors should be identified and corrected by the student. Instructor will stop commenting on these types of errors when any one page has more than 7 errors noted or when 10 total errors have been identified. Students are expected to improve in writing and seek support as necessary. Third person narrative.All components of the Problem Statement must be written in 3person.
Professional in appearance.Submissions should be presented neatly, in order, include all components, and be in black ink.
Avoid use of directly quoted material.Direct quotes are not recommended in the problem statement. If absolutely necessary, direct quotes must be properly cited and should be no more than 40 words.
Resources must be reliable, relevant, and recent. A minimum of five resources are required, with a mix of recent (published within the last 5 years) peer reviewed journal articles, governmental and organizational publications and reports.
Original writing.An original problem statement that uses the authors own words and credits the resources that are used to inform the authors knowledge and perspective.
