So, up to this point, you have identified an issue within a healthcare setting that is supported by data that has room for improvement. You have begun to look at the research evidence and anecdotal evidence to determine the nature and scope of the issue to determine why this particular issue needs to be addressed. It is highly encouraged that you are simultaneously addressing each of these issues in the classroom as well as working on your final paper to tackle each section as it is discussed. It will be much less daunting to write your final paper at the end! You have begun to look at the literature to determine what potential interventions exist that could address the issue you have selected. Now, we will take a more in-depth look to the evidence to select that one intervention that is evidence-based and will fit in to organizational resources and patient preferences to be your selected intervention(s) for implementation.
This week and next week, you will be completing a review of the literature. For your final paper, you will need a narrative review of the literature section. In NURS340 you searched and critiqued research. Here you will do the same, as well as synthesize the body of literature you found on your topic, identifying only those articles that specifically pertain to your topic and can support your intervention. Remember, to be relevant and useful, studies do not have to be randomized-controlled trials and meta-analyses, although these are still the gold standard of highest levels of evidence. In your search, look for smaller studies or write ups by organizations on their experiences with a QI or EBP practice change. These will help provide you with ideas, structure and guidance and you move on to creating your implementation plan. These articles provide valuable insight from those in similar settings who have been there, done that and can help you avoid potential pitfalls. These articles should not be used alone, but in conjunction with the higher levels of evidence that supported the intervention or practice change in the first place.
Articles that discuss one organizations experiences and outcomes implementing EBP or QI would fall under Level VI evidence. Remember that some topics (those that lend well to a more qualitative nature- patient satisfaction perhaps) will have a lower level of evidence rating. As you do your critical appraisal, remember that you are critiquing these studies for how well they were designed, performed, and their ability to be generalized to other settings. A good quality Level IV study is of more value to you than a poorly written Level II study. Only use those sources that you feel comfortable in the research process, methods and findings.
For your project/paper, you will need to find, summarize and evaluate at least seven current and relevant research articles. You may also use non-research articles/resources as needed to help provide context to the background of the situation or problem, but need to identify 10 research studies that would fall into one of the seven categories above. Studies should be current, and not older than 5-7 years, from peer-reviewed sources.
