Mr. Tamoe, 80 years of age, was recently admitted to the long-term care (LTC) facility on January 26, 2021 from his own apartment. Health history includes angina, hypertension, and osteoarthritis. Medication is taken for all 3 medical conditions. His daughter reported no history of dementia. Also, no reported allergies. He had a previous fall one month ago in his apartment- no injuries were sustained at that time. Mr. Tamoe has a hearing aid, glasses, and a walker for ambulation. A falls risk assessment was not completed at the time of his admission to the LTC. Mr. Tamoe was found on January 28, 2021 by the night nurse at 6:00 am when completing final rounds before the end of the shift. He was lying in the hallway just outside his private room. He was found wearing his pajamas and having not worn shoes or slippers. Mr. Tamoe did not have his hearing aid in, and he was not wearing his glasses. His walker was found beside him in the hallway. He sustained a small laceration to his left frontal area (forehead) and reported his left hip was painful, rating his pain as an 8 out of 10. Mr. Tamoe was alert and oriented to time, person, and place. Vital signs: BP: 150/ 90, Pulse: 94, Respirations: 24, Temperature: 37.
