Introduction: The processes in a clinical microsystem are the systems, processes and steps that are repeated with each patient who enters and receives care in the microsystem. The lack of knowledge about how these processes actually work, or how the same process may vary from shift to shift or with a different mix of staff, contributes to much of the waste, poor reliability, and errors within the microsystem.
Clinical Microsystem Assessment #2: PROCESSES. (Minimum 2 pages of content; Maximum 4 pages; APA 7th edition format). There are three parts to this assessment:
1. Identify and explain two processes that routinely occur as part of providing care to patients in your clinical microsystem. One of the processes identified should be standardized; the second should be a process that is not standardized. Please include evidence of the standardized processes (attach the policy or clinical guideline used as an Appendix… this way you can summarize it in the paper and the detail will be available in the Appendix). Discuss the process that is not standardized in detail, so that you can identify points in the process where ‘everyone does it differently’ (and how this may create inefficiencies in patient care, problems with patient safety, or different patient outcomes. See requirement #2 below). Consider using a flowchart as a guide (see Cherry & Jacob, p. 379). Please note that you are not required to complete a flowchart, but this will help you to identify the level of detail needed to describe the non-standardized process.
2. What is your perception of the effectiveness of each process? If it is a standardized process, is the standard implemented the same way by all shifts and mix of assigned staff? If it is not a standardized process, would standardization help make the process more effective? Justify your answer with experiential examples and evidence from the literature (See #3 below).
3. Use a minimum of two current (within the last five years), peer-reviewed NURSING journal articles to support the content presented in #2 (your perception of the effectiveness of standardized v. non-standardized processes).
Examples of clinical microsystem processes may include how patients enter the microsystem; how they are discharged to home or transferred to another unit; when patient education occurs; how shift or break ‘handoffs’ are implemented; how medications are administered; mechanisms for securing consultations; how diagnostic test results are scheduled, received and reported; how nurse: patient assignments are made… and many more!
