I wonder what the effect insurance reimbursement systems has on resource allocation for those patients who have traditionally experienced health care inequity prior to the PCMH model? Tao, et. al (2016) found that empirical studies are further needed to address how resource allocation and reimbursement systems should be designed in order to best serve those with greater health care needs and reduce health care inequalities.

As we all know that we do not have the best healthcare system in this country, having healthcare that is drifting towards a value-based service model that guides care in a primary provider to concentrate on continuity, coordination, and health disease prevention. On a personal note, I always feel having a value-based pay system will reinforce healthcare providers to provide quality care to their patients. Family practice nurse practitioners are very well fit in providing that type of care, for an NP’s primary role of care is based on a patient-centered model. Many physicians are leaving primary and focus more on specialized care because they do not equip to move towards value-based payment, according to Cuenca, 2017. Because of the patient-centered foundation that nursing has, an NP will have no problem building relationships with their patients. Once a rapport is established with a patient, it is easier to assume compliance with preventative care. Nurse practitioners are more compassionate to patient needs and concerns (Shi, Lee, Chung, Liang, Lock, & Sripipatana, 2017).
Therefore, nurse practitioners will have better outcomes than physicians. I have a co-worker who said that she would not trade her nurse practitioner for a physician because her nurse practitioner is extensive with her assessment and takes the time to review all lab results and point out any concerns. The relationships established between nurse practitioners and their patients will serve practitioners well in patient satisfaction survey scores. Nurse practitioners play a significant role in the office as well as the healthcare system general. NPs provide exceptional care at a lower cost, which allows more patients to seek health care. With a value-based pay system, NPs will bring and keep more patients in the clinic. Because the NP’s foundation is based on a patient-centered model, the satisfactory quality care levels will increase (Cuenca, 2017).
