Leading Quality Improvement Change Efforts in Healthcare Organizations and Facilities As healthcare administrators and managers continue to work to improve the quality of organizations and facilities served, you are often faced with an array of guidelines, best practices, and new tools that need to be incorporated into your setting to improve quality. These guidelines and best practices are often driven by accreditation, licensure, and certification standards from government agencies, not-for- profit agencies, and professional associations. At other times, the required change is driven internally by findings that emerged from the continuous quality improvement processes and measures engineered by healthcare organizations themselves. To successfully implement these changes, healthcare administrators and managers must develop change management skills that are essential for planning and implementing change in healthcare settings. This week will focus on understanding change in healthcare settings and learning about key strategies to help drive change.
 

 
By reviewing notes on an important health behavioral model, diffusion of innovation (Kaminski, 2011), you will garner a deeper understanding of how innovation or change efforts are adopted in healthcare facilities.

As shown in figure 6, about 15% of your target audience will be your early adopters of change—with 2.5% as innovators and 13.5% as early adopters. The bulk of your target audience will be in the early majority and late majority phase of the model, while 16% will be the last to adopt the change being introduced.
Through this framework, you will begin to appreciate why some change efforts fail and others succeed—and why uptake of certain change efforts occur faster than others. You’ll also examine the three key themes common to most change efforts:
1. Perception and attributes of change. 2. Characteristics and behavior of adopters.

This week, you will have the opportunity to garner best practices—such as understanding the driving and restraining forces, otherwise called the force field analysis—when implementing change efforts in healthcare organizations. Specifically, you will get insight into strategies that employ the use of continuous education as incentives, those that use printed materials in dissemination, and those that are delivered live.
In addition, you will learn about the role of opinion leaders (for example, respected physicians) as well as learn about strategies such as academic detailing, the use of reminders, and those that provide feedback to clinicians. More so, important strategies that involve the use of checklists, disease, and population management systems; decision support; and those that consider the use of financial incentives will be discussed.
